Office Policies 

  • Payment: Payment is expected at time of service. Please send payment in with a patient that is unaccompanied by a parent or legal gradian at the time of each visit. We accept check, Visa, Master Card, or Discover. For security reasons. we are not able to keep credit cards on file.

  • Late Payment: Please note visit that are unpaid for greater than 30 days will incur an additional $25 fee.

  • No Show Policy: One no show per family will be permitted without a charge. Any further no shows or same day cancelations without medical illness will be charged the full appointment rate for the missed appointment(s).

  • Refills: Please allow 24 business hours for refill requests. Please call the office rather than the pharmacy for refill requests.

​

  • Class II Medications: Class II stimulate medications are provided at the time of follow up appointments. Due to federal guidelines, all patients must be seen at least every 3 months to be eligible for class II refills. Please allow 48 business hours for class II refill requests that are outside of office visits. Photo identification and signature are required at the time of pick up in office by parent or legal guardian.
